Your treatment at Holbert Family Orthodontics is an investment that will last a lifetime, and we do not want confusion over financing and insurance to hold you back. Orthodontics will improve your dental health and also enhance self-esteem and self confidence. This is especially important for children who may be having a difficult time at school or in social settings.

We understand that orthodontic treatment is a significant financial commitment. This is why we provide our patients with multiple payment options. Our financing and insurance options are designed to provide the best options possible for our patients. We want to make orthodontic treatment affordable for you and your family, so please feel free to ask us about alternate payment arrangements and if you have any questions in general about financing and insurance.

Questions about financing & insurance are common when it comes to orthodontics, and our team is here to make it as painless as possible. our orthodontic investment includes all office visits and procedures provided by Dr. Holbert over the estimated treatment time. Our office will provide all appliances required during orthodontic treatment, as well as your first set of retainers and 18 months of monitored retention care. We want to provide affordable highquality treatment that fits your lifestyle.

Yes. We understand that orthodontic treatment is a significant financial commitment, so we’ve done our best to offer plans that are both convenient and affordable. We have multiple financing & insurance options available to you.

We have created financing & insurance payment options in an attempt to meet every patient’s needs, but we are always willing to discuss other arrangements that may be more suitable for your personal situation.

There are many different agreements between insurance carriers and their subscribers. Each contract provides a different level of benefit. Orthodontic insurance differs from regular dental insurance in that each insured individual usually has a lifetime maximum benefit.

The benefit is paid as a percentage of the orthodontic fee until the maximum is reached. Insurance payments are made over the course of treatment as long as coverage is in effect. Charges for lost or broken appliances are generally not covered by insurance.

As a convenience to you, we will gladly submit insurance claims for charges pertaining to care provided by our office. Please be aware, however, that our primary financial relationship is with our patients and their families and not with their respective insurance companies.

Financial arrangements can be made based on your estimated insurance benefit; however, any outstanding insurance claims not paid are the responsibility of the patient or the patient’s family.

To assist us in handling your insurance claims, we ask that you provide the name, address, and telephone number of your insurance carrier. We are unable to submit your claim until you have signed the authorization for information release and the assignment of benefits located on the Financial and Office Policies Agreement Form. You do not need to bring an insurance form to our office.

When it comes to financing & insurance at Holbert Orthodontics, we are committed to making the process as smooth as possible. Once we know the estimated amount expected from your insurance carrier, we set up a separate section of your account specifically for insurance payments. Although you are acknowledging your financial responsibility for the entire orthodontic fee, the financial contract is structured for the estimated amount due from you.

Insurance payments are posted to the insurance contract on your account and personal payments are posted to your personal account. There are a few carriers who will only send their payments directly to the patient. These accounts are structured with the entire balance on the patient contract.

Please notify our office if you become aware that your insurance benefit has been discontinued or changed. We will notify you if we receive information about a change in benefits. Any amount of the estimated insurance benefit the carrier does not pay will be added to your remaining contract.

We know that financing& insurance changes happen. Any change in insurance benefits, discontinuation of insurance benefits or processing that results in an outstanding balance are the responsibility of the patient or the patient’s family. The additional amount due will be divided over the remaining scheduled payments and your monthly fee will be increased accordingly.
