NEW PATIENT FORMS

We ask that you take a few minutes to print and fill out the new patient forms before your first appointment:

  • Patient Form Packet (Child) PDF – Your child’s health history is important to us. Certain medical conditions, allergies or medications could affect the type of orthodontic treatment that is appropriate. This history also helps Dr. Holbert assess any potential risks or complications that may arise during treatment, enabling him to tailor the treatment plan specific to your child’s needs.

  • Patient Form Packet (Adult) PDF – Just like with children, adults may have underlying medical conditions, allergies or medications that could impact orthodontic treatment. Adults may also receive orthodontic treatment as part of a comprehensive dental or medical care plan. Understanding your health history allows Dr. Holbert to coordinate care effectively with other healthcare providers, ensuring a successful and safe treatment.

  • Online Adult New Patient Form – Filling out your new patient forms ahead of time helps us streamline the check-in process at our office, saving time for both you and our team! You can complete your new patient forms at your convenience prior to your appointment, so it’s less for you to do when you get to our office and it helps minimize wait times.

  • Online Child New Patient Form – As a parent, you’re juggling enough as it is. Our online child new patient form makes it easy and convenient for you to fill out your child’s medical history, dental history, insurance details and specific concerns. Collecting this information on your new patient form in advance allows Dr. Holbert to review it thoroughly before your child’s appointment.

After you fill out your new patient forms, you may have questions about what else to expect. Your initial consultation is a great time to ask questions, express any concerns you may have and learn more about orthodontics including your treatment options. Here’s what you can expect during your first visit with us:

  • We will do a thorough review of your dental and medical history

  • Dr. Holbert will perform an evaluation of your mouth which includes: taking photos of your smile and your teeth, as well as X-rays if needed

  • A customized treatment plan is designed

  • Discuss financial information, insurance options and our payment plans

FREQUENTLY ASKED QUESTIONS

I’ve completed my new patient forms and had my consultation with Dr. Holbert. Now what? Following your discussion and evaluation with Dr. Holbert, he will write a treatment plan specifically for you. Our friendly team will then make your next appointment where Dr. Holbert will place your new appliance on your teeth and your smile journey officially begins!

It depends on your treatment. If you are wearing braces, you will most likely need to come into our office every four to eight weeks. This allows Dr. Holbert to make any adjustments, fix broken brackets and switch out your bands if needed. For our clear aligner patients, your appointments are usually less often–about every four to six weeks to make sure your smile is on track!

At Holbert Family Orthodontics, we understand things come up. If you’re behind schedule or need to come in another day, please give us a call and we will do our best to work with you on a new appointment time.

Yes. Your dental health is extremely important while undergoing your orthodontic treatment. By keeping up with your routine cleanings, it prevents food and plaque build-up on your teeth, which makes sure your gums stay nice and healthy.
